（1）被邀请人访问信息：来华事由、拟抵达日期、拟居留地点、拟居留期限、与邀请人关系、费用来源等；

（2）邀请人信息：邀请人姓名、联系电话、地址、邀请人签字等。

如系寄养事由，须提供：

1、中国驻外使领馆出具的寄养委托公证或经过所在国或中国公证认证的寄养委托书；

2、委托人的护照原件、复印件及其与被寄养儿童的亲属关系证明（结婚证、出生证、公安派出所出具的亲属关系证明或亲属关系公证书等）；

3、接受寄养的受托人出具的同意接受寄养函件及身份证复印件；

4、寄养儿童的父母双方或者一方为中国公民的，还应提供儿童出生时中国籍父母在境外定居证明的复印件。

另外，Q1签证及S1签证持证人入境中国后，须在30日内向拟居留地县级以上地方人民政府公安机关出入境管理机构申请办理居留证件。

二、申请方式

Application method

1、中国驻新加坡大使馆不直接受理持普通护照者的签证申请。持普通护照者申请中国签证请到中国签证申请服务中心办理。申请可由本人或委托代理人、代理机构帮助递交（无需提供代理委托书）。

2、使馆认为必要时，可要求申请人本人前往使馆接受面谈。

3、使馆不接受其他方式（如邮寄、传真、网上）的签证申请。

4、除非使馆另行通知，申请人或代理人最早可于取证单上标注的时间前往签证申请服务中心领取办妥的签证。不提供邮寄签证服务。

5、签证申请一经批准，不可撤销。申请人需支付相关费用。
