個人簡歷
Resume/CV
請將最新個人簡歷（必須包含教育背景、工作經驗、所屬協會或組織、個人成就以及海外經驗）以及個人陳述視頻版的URL連結等上傳至申請平台。
Please upload your latest/updated resume/CV which MUST include your education background, work experience, memberships in associations/organizations, achievements, international experience, URL link to the video submission of your response to the personal statement etc.

身份證明文件
Official Identification Documents

Please upload the following documents on the application portal:
1） 如申請人為新加坡公民或永久居民，請上傳護照或身份證正反面掃描件；
A scanned front and back copy of your NRIC or passport if you are a Singapore citizen or Singapore Permanent Resident；
2） 如果海外申請者申請全日制或非全日制項目，請上傳護照掃描件；
A scanned copy of your passport if you are an international applicant applying for either full-time or part-time programme；
3） 申請非全日制項目的海外申請者，還需上傳本人新加坡就業准證Employment Pass/Work Permit/S-Pass正反面掃描件；
A scanned front and back copy of your Singapore Employment Pass/ Work Permit/ S-Pass if you are an international applicant applying to part-time programme；
4） 護照照片掃描件；
A scanned copy of passport photo
僅接受.jpeg格式文件；

文件大小不得超過125kb；
Less than 125 kilobytes in file size
背景須為白色；
Taken against white background
照片尺寸為340像素*453像素（長寬比為0.75）；

註：持有就業准證的申請人只能申請在職項目
Please note that if you are on employment pass, you are only eligible to apply for the programme (part-time).

學位證書及成績單
Official University degree scroll and transcripts/mark sheets
請在網絡申請系統中的「文件上傳」區域上傳每個學習階段的官方或經官方認證的學位證書以及成績單。
Please upload your official or officially certified copy of diploma/degree certificate and transcripts/mark sheets of your academic progress at each education institution attended to 「Documents Upload」 on the online application system.
除非上述由學校發布的學位證書及成績單為英文，否則需要在源語言材料之外附加一份經過認證的英文版本。
Unless academic records and diplomas are routinely issued in English by the institution, the official records in their original language must be submitted with an authorised translated English version.
所有的官方成績單都應註明入學及授予學位的時間、學位名稱、所修科目或課程以及每門課程的學分。如果有排名也請註明。文件還應包含一份完整的對於學校成績評定或其他評價標準的介紹，包括最高分、最低分以及其間的梯度。
All official academic records should show the dates of enrolment, conferment, original title of degree, the subjects or courses taken, together with the units of credit allotted to each subject and, if available, please indicate your rank. The records should also include a complete description of the institution's grading scale or other standard of evaluation. Maximum, minimum marks and the steps between them should be indicated.
你僅需提交在「教育背景」部分所列出的學校的成績單即可。
You need only to supply transcripts from schools you list in the 「Academic Qualifications」 section.

GMAT或GRE成績
GMAT/GRE Test Score
請依據不同考試類型，將官方成績報告上傳至申請網站。如果你有多次考試紀錄，請於申請截止前上傳最近一次的分數。僅接受5年內的考試成績。
Please upload your official score report by the testing institutions on the application portal. If you have 2 sets of scores, please upload the latest before the application closes. Test score will only be accepted if it is taken within the last 5 years.
申請人必須在申請頁面的「其他考試」區提交GRE或GMAT考試成績及百分位。如果無需提交GRE或GMAT成績，請在「其他考試」部分輸入零。
You MUST provide your GRE/ GMAT score and percentile in the application portal under the "Other Tests" section. If you are applying without GRE/GMAT, please put input zero in the 「Other Tests」 section.
如果申請人為下列任一所新加坡本地大學的理工、數學、統計學、工程學、計算機、商學或經濟學專業的畢業生且獲得學位，即可豁免提交GMAT或GRE考試成績。
You are exempted from GMAT/GRE test if you are a graduate with degree in Science, Mathematics, Statistics, Engineering, Computing, Business or Economics from either of the following local universities:

兩封推薦信
Two referee reports
聯繫兩位推薦人，且確保兩位推薦人分別在錄取截止日期前提交推薦信。
Please ensure that you have approached two referees, each to submit one referee report to us before the admission deadline.
兩位推薦人可以是申請人的導師、現任或前任僱主，或是公司客戶。他們需要能夠提供關於申請人能力、成就或潛力的具體信息。
The two referees can be your academic supervisor, current or previous employer, or corporate client as long as they are individuals who are able to give specific information about your abilities, accomplishments and potential on the application portal.
申請人在網絡申請系統提供兩位推薦人的具體聯繫方式。這一信息十分重要，學院將通過該途徑與推薦人取得聯繫。請將參考表格發給你的兩位推薦人，提醒他們在你完成網上申請之前提交推薦信。
In the online application system, please provide accurate contact information about your referees. This information is important as it allows us to contact your referees. Please initiate the online referee form to your two referees to remind them for submission before you complete your online application. Your referee will receive an email containing instructions on how to access and submit the online referee report.
